Supplemental Disclosure for Organizational Conflicts of Interest (OCI)
The Supplemental Disclosures for Organizational Conflicts of Interest (OCI) is a U-M form that principal investigators (PIs) must complete, in rare circumstances,when OCI requirements are found in requests for proposals. This form helps ensure U-M is compliant with sponsor requirements to identify and address any potential OCIs.

When Should This Form Be Completed?
The Supplemental Disclosure for OCI form must be completed by the principal investigator (PI) on behalf of themselves, the project team, and the PI’s and project team’s family members when grant and contract proposals and awards (including subcontracts) to U-M include an OCI requirement. The form needs to be completed before the proposal is routed to the Office of Research and Sponsored Projects (ORSP). A COI Office will review the form and issue a determination.
If a proposal approval form (PAF) is submitted to ORSP that has OCI requirements in the sponsor guidelines and a COI Office determination regarding OCIs has not been made, ORSP will return the PAF with a required change for the PI to complete the OCI form and obtain a determination from the COI office before returning the PAF to ORSP for submission.
How Is This Form Different From M-Inform?
The M-Inform annual outside activity disclosure and certification process captures individual outside activities, relationships and interests. This supplemental form captures information related to projects and other activities inside the university and reflects the information most often requested by sponsors regarding OCI.
